Mental Health Social Worker - Treatment & Recovery X2

The London Borough of Hammersmith and Fulham is looking to recruit two motivated, enthusiastic Mental Health Social Workers/ Approved Mental Health Practitioners to join our Treatment and Recovery Team.

The post offers an exciting opportunity work as part of a multi-disciplinary team which is at the forefront of modern mental health service delivery. You will work as a care co-ordinator with adults and older people with severe and enduring mental illness. You will work collaboratively with our clients and their families to support them in their recovery journey. The successful applicant will work closely with our in-patient services, home treatment and recovery Teams, CAMHS and primary care settings, in addition to voluntary and statutory agencies, the police, forensic and probation services. 

You will have experience of working as a care co-ordinator in a multi-disciplinary environment and have excellent written and verbal communication skills. You will have a sound knowledge and experience of the law relating to mental health and social care. You will be skilled in carrying out assessments, including risk assessments, and creating and co-ordinating care plans and CPAs.  AMHP applicants will be expected to  carry out assessments under the Mental Health Act 1983.

You will receive the support of a multi-disciplinary team, team leader and service manager. You will receive regular supervision and support for your professional development. Hammersmith and Fulham are committed to providing a sound, supportive framework for Approved Mental Health Practitioner training and practice.

Applicants should hold a Social Work qualification, up-to-date registration with the HCPC and have experience of working in a mental health community setting. You will be subject to a DBS check.
